Insurance and Payments

We accept several types of vision insurance plans, medical insurance, cash, checks, and most credit cards. We inform our patients about expected exam and treatment costs at the beginning of your appointment. If you require a breakdown of costs, please let us know. Insurance is not required for us to see you. Ask us about same day/self pay payment discounts.

Insurance Coverage

The knowledgeable staff at Mt. Juliet Family Vision is experienced in working with various insurance plan providers. We will work directly with you and your plan to submit claims regarding your Eye Health and Vision care. We strive to ensure that you understand your plan coverage and will answer any questions you may have regarding your benefits. We accept most Major Medical Insurance plans and several Vision Plans. If you have or there is an Eye Health related issue, your exam can be billed to your Medical Insurance Plan. If you are in for a routine examination with no Medical concern, your exam can be billed to your Vision Plan. Eye-related issues such as cataracts, glaucoma, Macular Degeneration, pink/red eyes, Dry Eyes, among others will be billed to Medical Insurance for proper eye care and treatment plans. Vision Plans can always be used in the optical or for contact lenses. Vision Plans are designed to cover many of the basic vision-related costs, but not all costs associated with eye care and lens enhancements- we can help you understand your plan to provide the clearest vision possible. We are not in charge of your plan's copay amounts, unmet deductible amounts, nor what your plan may describe as a non-covered service. We do our best to get insurances to cooperate and pay correctly, but it is ultimately your insurance plan and responsibility.

Vision Plans: Some plans we accept: VSP, Eyemed, Cigna Vision, Superior, VCP.

Medical Insurance: Some Medical Carriers we accept: Medicare, BlueCross BlueShield, Cigna, Humana, United Healthcare, Aetna, UMR, UHC RiverValley, HealthScope Benefits

*Contact our office at (615)758-2501 to find out if your insurance provider is accepted in our office, as it can vary or for more information about your vision plan or medical insurance coverage*

*If we are Out of Network (OON) with your vision plan provider, you can still collect your OON benefits for exam and materials by submitting simple forms online.

Flexible Spending Accounts (FSA) and/or Health Savings Accounts (HSA)

Many employers are offering Flexible Spending Account (FSA) options to employees or you may have a Health Savings Account (HSA). Sometimes these are referred to as cafeteria plans and are elective, supplemental insurance savings plans. These plans are designed to let you save money in an account, pre-tax, to pay for additional medical expenses such as eye exams, glasses, contacts, and often laser vision surgery. Check with your insurance benefits administrator at work to see if you are eligible for this program. FSA/HSA's can typically be used in conjunction with any vision plan or medical insurance to offset any out-of-pocket costs on your behalf.

Other Payment Options

Our Mt. Juliet Family Vision staff understands that cost is a concern when evaluating vision care providers. We do not want cost to be an obstacle to maintaining proper eye health and good vision. We accept various forms of payment and are able to discuss financing options with you prior to any examination. At Mt. Juliet Family Vision, we also accept cash, check, and major credit cards.
